Survey Engements for PLS exam sitting

Hi there.
I'm preparing my paperwork to take the test next year (yeah, the recently passing deadline of 5/1/12 got my tail in gear) and I have a question.
In my past years, i worked for a company for 5 years. during that time, we had one main PLS in our company and then other PLS's that i worked under while they worked there and i worked under the main when no other PLS was there as i took over some of the local office responsibility.
my question is, for my engagement record, do i have to fill out and have our main PLS sign multiple forms for each time i started working under his indirect supervision?
ie, year 1 i worked directly under PLS A
year 2 i worked indirectly under PLS Main
years 3-4 i worked Directly under PLS B
year 5 I worked indirectly under PLS Main
all was with the same company i just had different bosses. So, I believe I would have 4 different engagement records and 3 signatures during that time period, right?
my next question is: does the board look at if the reference was my supervisor or my co-worker differently? do they count my time any differently?
